Parish Verification Forms
February 23rd, 2009The Parish Verification Forms are included in this week’s Tuesday packet for Holy Spirit and Newman Center parishes. If you attend one of these parishes or have changed parishes and did not receive a form, please contact Mrs. Reynolds in the school office as soon as possible. You do not need a form if you are registered at St. Anthony’s unless your status has changed. Please fill out the form and give it to your parish office and they will mail it directly to our office upon completion. This form must be returned by Monday, March 31st in order to receive the active Catholic rate on your school contract for 2009-2010. Please remember that your St. Anthony’s tuition rate is based upon your prior year’s attendance at your parish.
